You will need our Principal group number, which is 1020916. Once registered, you can create the following documents:

Durable Power of Attorney
Power of Attorney for Health Care
Medical Authorization to Treat Minor
Identity Theft Affidavit
These documents can be printed and notarized. They will be saved and available to edit if there are any changes to your information. A full educational library is on the site and provides information on topics such as probate and estate taxes as well as a description and purpose for each document. There is also a Personal Information Organizer, which allows you to keep all of your important information in one document, along with your wishes related to funeral arrangements.
