BCBS Pharmacy Changes

Effective March 1, 2012, there are a couple of changes to BCBS prescription drug formulary:

 

Moving from Tier 3 to Tier 2 (copay reduces from $70 to $35):

 

Victoza - Insulin Product

Tradjenta - Diabetes

 

Moving from Tier 2 to Tier 3 (copay increases from $35 to $70):

 

Apidra - Diabetes

OsmoPrep - Bowel Preperation

 

Medications requiring step therapy (must try and fail a generic alternative before a brand name will be approved):

 

Nuvigil must be tried before Provigil - narcolepsy, sleep apnea

Fibrate Class  (such as Bezalip, Modalim, Lopid, TriCor) - cholesterol

Topical Acne

Topical Corticosteroids - eczema, rashes

 

Drugs requiring prior authorization:

 

Suboxone - Addiction
